How to express concern or complain formally

You can express your concern in any of the following ways:

• Use the form on our website at https://www.aneurinleisure.org.uk/concerns-and-complaints

• Ask for a copy of the form from the person with whom you are already in contact. Tell them that you want us to deal with your concern formally.

• Get in touch with our Business Support Manager on 01495 369691 if you want to make your complaint over the phone.

We aim to have concern and complaint forms available at our facilities. Copies of this policy and the complaint form can be made available in Welsh upon request.
